Requesting Imaging Records

CHI Mercy Health utilizes PowerShare, a complete image-sharing solution that enables us to securely and quickly share medical images with patients and providers.  We provide free and secure online access to your diagnostic images.

PowerShare offers immediate access to patient images, allowing them to be viewed from anywhere, anytime, using any internet-connected device.  This service eliminates the need to request CDs for image access, providing a more convenient and efficient solution.

As of August 15, 2024 we will no longer be providing CD’s.

Patients Requesting Images

Via Email

Please email the imaging department at [email protected] with the following information:

1. Name
2. Date of Birth (DOB)
3. Study Date
4. Exam Description
5. Email Address to Send Images To
6. Phone Number
7. A Picture of your Photo ID

Please ensure all information is accurate and complete to process your request promptly.

Provider offices or other Facilities Requesting Images

To request patient images, please follow one of the two options:

1. Email: Send an email to the imaging department at [email protected] with the required information.
2. Fax:  Fax your request directly to the imaging department at (541) 677-2351.

Please ensure to include all necessary details for a prompt response.
