Thank you for applying to become a volunteer at CHI Mercy Health. Here are the next steps to joining our amazing team of volunteers.

Step 1:  Please complete Background Check Authorization Form Below.

Step 4: Using the information from the Volunteer Orientation Handbook and Our Values in Action Policy and Reference Guide, please complete the HIPAA and Safety Tests below.

Step 5: Health Requirements – TB Tests

All NEW Volunteers are required to have a TB test completed. There are some exceptions to this, based on your TB test history. TB testing and flu shots are offered by our Employee Health department at no cost to you. Upon submission of this Orientation Packet, you will be contacted by the Volunteer Resources Office to schedule your TB test and meet to review placement.
